- Participants are seen by appointment only. Attached are instructions on how participants can sign up for their appointments.
- Fill out the COVID-19 Immunization Consent Form prior to your appointment
COVID Vaccine Informed Consent (Fillable).pdf - Bring their Drive License/ID, medical and prescription insurance card to the appointment
- Bring proof of eligibility
- Wear short sleeve shirt if possible
- Do NOT attend clinic if you are feeling unwell or experiencing COVID symptoms
NEW WAY TO SCHEDULE A VACCINE APPOINTMENT Starting today, residents can be notified by email or text when they become eligible for a vaccine AND when an appointment becomes available! Signing up for the service is simple and takes less than 5 minutes. You'll get an email or text confirming your eligibility and another email or text when an appointment is available. It does not work with landline phones. Visit My Turn for more information.

INFORMATION ABOUT SECOND DOSES FROM COUNTY PUBLIC HEALTH OFFICES
- If you received your first dose of Moderna from the Public Health Placerville office, you should have received an email with information about how to register for a Moderna second shot only clinic to be held February 18th. If you haven't received an email invitation to schedule your second dose, please contact eochealth@edso.org. Emails to those who received their first dose from the Public Health office in South Lake Tahoe will be sent by 2/19/21 for upcoming clinics.

BY APPOINTMENT ONLY at
El Dorado County Public Health Department in Placerville and South Lake Tahoe and
select Safeway Pharmacies for all categories in Phase 1A tiers 1-3, Phase 1B tier 1 and anyone aged 75 and older regardless of employment status. (Register for one only.) WALK-INS WILL BE TURNED AWAY.
- Appointments are available based on the amount of vaccines allocated by the State. If there is no vaccine available, no registration for an appointment will be possible.
- Proof of residency in El Dorado County, employment status in El Dorado, and/or age may be required at time of appointment.
- Please be aware that the Pfizer vaccine cannot be administered to persons under 16 years of age; Moderna vaccine cannot be administered to persons under 18 years of age.
- There has been significant interest across the County from the people who are eligible to receive a vaccine. Please note that clinics are only allowing appointments to be scheduled based on how many doses they have available. Currently, appointments are full through mid-February at the County's Public Health offices and all six Safeway pharmacies.

CVS/Rite Aid (Moderna)
The Federal Retail Pharmacy Program has been activated, allowing select store locations to begin providing Moderna vaccine by appointment only. Eligible populations include residents 65+ and healthcare workers. Appointments are required. Eligible residents should not contact individual stores for appointments or come to the store seeking a vaccination without an appointment.
In El Dorado County, the following CVS store is currently providing vaccinations:

Public Health vaccinators cannot vaccinate persons who:
Are exhibiting COVID-19 symptoms
have had any vaccination in the last 14 days
Pregnant or breastfeeding women
have had an anaphylactic reaction to a vaccination or injectable
passive antibody therapy in the last 90 days.
