MULTIPURPOSE SENIOR SERVICE PROGRAM
The Multipurpose Senior Service Program (MSSP) is a voluntary care management service. The program provides care management, which includes information and assistance about appropriate community resources to meet needs, and monitoring of the client's on-going care and health status. A care manager and a public health nurse visit the client in his or her home, meet with family members as needed, and work with the individual to create a plan of care so they can remain at home. On-going and frequent contact between MSSP staff and participants ensures that necessary care is provided.

The MSSP enrollment plan begins with a comprehensive lifestyle evaluation. After assessing the client's needs, the care team works closely with the client to determine what they need to remain safely at home.
