HeartFAMILY CAREGIVER SUPPORT PROGRAM

Promoting Family Caregivers Through Support and Care

El Dorado County, Family Caregiver Support Program (FCSP) is designed to offer assistance to persons caring for an older adult (60 years of age or greater) or a grandparent (60 years of age or greater) who is raising a grandchild or other kinship who is 18 years of age or younger.

Did you know the 60+ age population in El Dorado County is growing at three times the national average? And that the 75+ age group, the most vulnerable, is growing even faster? In response to these growing numbers and to the people in our community who suddenly find themselves caring for a family member, we offer the support you need to provide the best care.

Who Qualifies for FCSP?

The informal caregiver must reside in El Dorado County
The care receiver must be 60 years or greater OR
Grandparent or relative caregiver must be 60 years or greater and be caring for a child 18 years or younger

FCSP Services

Information and Referrals

Accessing Services / Care Management

  • FCSP professionals provide a comprehensive caregiver assessments or case management through a home or office visit and telephone call follow-up. This assessment will determine the necessary supportive or other appropriate services to meet the caregivers needs.

Caregiver Education, Support Groups and Training

  • FCSP provides caregiver education, trainings, and support groups to assist caregivers in sustaining a healthy experience both physically and mentally. FCSP also provides referrals to trained and/or licensed professionals to assist caregivers with problem solving and emotional support.

Supplemental Services

A variety of other services may be provided by the FCSP to qualified caregivers on a limited basis. Those services include:

  • Minor Home Modification
  • Placement
  • Homemaker
  • Chore
  • Home Security and Safety
  • Assistive Devices
  • Visiting
  • Legal Assistance
  • Peer Counseling
  • Income Support / Material Aid
  • Money Management

Respite Care

  • The FCSP provides temporary relief for a brief period of time. Respite can be in the form of in-home respite or day care respite.
